A statement of use will need to be filed if you originally filed as an intent-to-use. The statement of use is simply telling the USPTO that you are now selling your goods/services across states lines (interstate commerce) OR between the US & a foreign country. Be sure to keep out for the NOTICE OF ALLOWANCE — you only have 6 months from the date it was sent to either file the statement of use or file for an extension.The jpeg attachment is to attach your specimen — this is visual proof, if you will, of your mark in commerce. For instance, if you’re selling a product, you could send a tag, label, etc. If you’re selling a service, you could send an advertisement, a brochure, etc.I posted some links that should help you further.
